Weekly meetings produce quick decisions, but action items often live in personal notes. By the next meeting, the team remembers the discussion but cannot tell whether the work moved. An action-aging review gives each commitment a visible owner and closing test.

Define the record before the shift starts

Write the action as a verb plus an observable result. Record the decision source, accepted owner, due date, dependency, and evidence expected at closure. Avoid entries such as follow up or handle vendor. Those phrases conceal what completion means.

Work from the controlled source

Before each recurring meeting, sort open actions by age and dependency. Ask owners for a factual status, not a percentage guess. If a task depends on approval, access, or source material, name that dependency and its owner. The meeting can then address the real wait instead of replaying the original conversation.
